Absence Management / Frontline

All administrators, teacher and instructional assistant absences are to be entered into our Frontline Absent Management /AESOP site so that we can keep track of employee absences in a central location that also initiates the process of securing a substitute teacher.

Human Resources Staff

The Human Resources department is committed to enhancing the work environment for our valued employees. We support the mission of the Fond du Lac School District by administering any employment-related processes, overseeing and coordinating District relations, and working directly with the superintendent on reviewing, developing and implementing the policies of the District.

COVID

Safety remains a priority to our school district. If you have tested positive for COVID, a report should be made to the Director of Human Resources so that we can monitor the impact COVID may continue to have on our people within our system. Government-sponsored COVID leave ceased in June of 2021, and our district discontinued the extension of this leave in June of 2022. Our district will waive the need for a doctor's note for an extended absence of 5 days or more when the absences are related to a positive COVID test; we plan to adhere to the current CDC guidelines and honor the need for a 5 day quarantine from onset of symptoms for anyone testing positive. This leave will come out of your allotted sick leave. Masks are not required upon return for days 6 - 10.

Remote work may be considered, upon request to the Director of Human Resources, in situations in which remote work falls within the scope of the employee’s job function. There will be no expectation for anyone to work remotely if they are not feeling well enough to do so.

If you are a close contact to someone with COVID, we ask that you monitor for symptoms.

Please continue to self-monitor and notify Marisa Lombardo and your direct supervisor if you seek a COVID test due to symptoms:

  • Fever (100.0° F or greater)

  • Chills

  • Muscle aches

  • Headache

  • Sore throat

  • Nausea or vomiting

  • Diarrhea

  • Fatigue

  • Congestion or runny nose

At least ONE of the following symptoms:

  • Cough

  • Shortness of breath

  • Difficulty breathing

  • New loss of smell

  • New loss of taste

Employee Assistance Program (EAP)

The Fond du Lac School District is pleased to partner with Agnesian Work & Wellness for our Employee Assistance Program (EAP). Our EAP is a worksite resource designed to enhance all of our health and productivity through prevention, identification, and resolution of personal and family issues that might interfere with work. Call 920-924-0614 for a confidential appointment. EAP is accessible to all employees and family members regardless of your insurance provider.

Employee Incident Form

All injuries and/or incidents must be reported to your building principal or supervisor and/or the school nurse. We encourage any professional medical attention for our staff to occur at our Occupational Health Clinic at Aurora, 920-907-7240. Anytime medical attention is sought for a Workers Compensation injury a Return to Work slip is required upon return.

Resignation

On behalf of the Fond du Lac School District, we would like to thank you for your service to the students and Fond du Lac community.  Official procedures need to be taken to end your employment with the Fond du Lac School District.  Please follow the steps below:

  1. Notify your principal or direct supervisor.

  2. Complete the Resignation Form (see below) and send it to the Human Resources office to give official notice.

  3. Complete the Check Out procedures listed on the Employee Separation Form (see below).

  4. Deliver your completed Employee Separation Form and items to be returned to the Human Resources office upon the commencement of your last day.

  5. Complete the EXIT SURVEY. If you would like a personal exit interview, please email Marisa Lombardo to arrange a meeting time: lombardom@fonddulac.k12.wi.us

Retirement

  4. Deliver your completed Employee Separation Form and items to be returned to the Human Resources office upon the commencement of your last day. Access to Google (email, docs, etc. will be turned off the Monday following the day of separation.

Substitutes

Do you know someone with a high school diploma who may want to earn $110/day by  becoming a substitute Instructional Assistant? Instructional Assistants help within many classrooms throughout the district and sometimes monitor our students while the teacher is instructing into the classroom remotely.
